The Connections For Life (CFL) program assists individuals with disabilities in becoming active members of their community and provides in-home supervision, while family members are present or not, for those served.

Key aspects of this program include the following:

  • Volunteerism at local non-profit organizations
  • Participation in social, educational, and recreational activities
  • Learning new skills in real-world settings
  • Maintain and support the stability of the family unit
  • Gaining valuable work skills
  • Developing new relationships

The Impact

  • Community integration was minimal due to COVID-19.
  • 83% of individuals with a health and wellness related goal were able to achieve those goals.

The Basics

Services are provided in the community or home of the individual served. Service hours can be up to 24 hours a day, 7 days a week. Services are provided for individuals of all ages.

The Details

When individuals with disabilities live as independently as possible, they have an increased quality of life and an improved attitude. The Connections For Life program aims to help individuals learn how to live as independently as possible. Gaining independence requires individuals with disabilities to:

  • Increase their life skills by learning to develop money management skills
  • Gain an understanding of local services and resources that apply to the individual
  • Be knowledgeable of how to use community resources such as banks, libraries, and public transportation

Financial difficulties, disruptions of family routines, and mental and physical fatigue are a few of the common stressors for families of an individual with disabilities. Connections For Life offers clients and their families the assistance needed to keep the family unit stable and intact.

The Connections For Life program has developed relationships with more than 20 local non-for-profit organizations willing to provide volunteer opportunities for adults with disabilities who, in turn, provide meaningful volunteer services. Volunteer opportunities allow individuals with disabilities a chance to:

  • Give back to their communities
  • Gain valuable work skills
  • Enhance social skills
  • Develop relationships with other volunteers who do not have a disability

Increased social interaction and the development of new friendships enhance the quality of life for those we serve. Furthermore, when individuals with disabilities share their skills, stories, and potential with their colleagues, friends, and neighbors; our community is enriched.

The Connections For Life program follows best practices in the field of service delivery for people with disabilities. The program is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF), whose purpose is to promote the quality, value, and optimal outcomes of services.
