Home Coordinator

Full-time position, Flexible schedule; weekends and evenings. The Home Coordinator assists in providing and managing around the clock in-home supports and assistance to individuals served including but not limited to providing supervision and training to Direct Support Professionals on daily living tasks, ensuring the cleanliness of the home and assisting with client grocery shopping, assisting with the creation and distribution of staff schedules, managing assigned residential sites, leading/participating in ISP or quarterly meetings, accompanying clients on medical appointments, ensuring all documentation is completed in a timely manner, and managing medications for assigned residential sites. The Home Coordinator will serve as a first-line response for emergencies and staffing needs and will fill in for any open shifts to provide monitoring, supervision, and active treatment for clients when a DSP is unavailable. Candidate must have a high school diploma/GED (degree or certification in Human Services or related field preferred), must be at least 18 years of age, provide proof of automobile insurance, and must successfully complete Core A and Core B Certifications. Experience with individuals with cognitive and physical disabilities is preferred.
